You can download a copy of the Joliet Police Complaint and Inquiry Form.  Once the form is completed, it can be turned into any member of the Joliet Police Department, mailed to Internal Affairs Unit at the Joliet Police Department located at 150 W. Washington Street Joliet, IL, 60432,  or emailed to JPDInternalAffairs@joliet.gov.
